Self-referral

Some health-board services can now be accessed directly, without a referral from your GP.

You can refer yourself to the following services by downloading, completing and returning their referral form, or contacting the service direct:

• Physiotherapy self-referral
• Dietitian self-referral
• Podiatry self-referral form
• Occupational Therapy self-referral
• Community Chaplaincy Listening Service
• Quit Your Way Hebrides (smoking cessation)
